By signing and submitting your form you hereby accept DFS’s terms and conditions and that you will also:

  • Make an initial payment of £200.00 which is non refundable in the event that you decide not to continue with your membership once your application has already been processed.
  • Make monthly premiums of £10.00 through the Direct Debit mandate.
  • Make a £10.00 payment every time there is a funeral of a member.
  • Get a cash lump sum of £10 000.00 in the event of a bereavement and upon submission of proof of death.
  • Not receive any bereavement payment for the first six (6) months from the initial signing up period.
  • That your plan will be cancelled with immediate effect should you miss a payment.
  • Only those covered in the plan at the time of death and having satisfied the six (6) months cooling off period will be eligible for the £10 000.00 bereavement cash lump sum payment.

Benefit/payout shall be as follows:

  1. 0-3 months – no payout (deferred/window period)
  2. 3-6 months – £2 500.00
  3. 6-12 months – £5 000.00
  4. Over 12 months – £10 000.00

Financial payout will only be made if the deceased person was a fully paid member for a period of three (3) months or more and their subscription being up to date.
